Pharmacokinetics

Glipizide belongs to the group of long acting hypoglycaemic agents. It is rapidly absorbed from the GIT, and is metabolized in the liver. It appears in the blood within 1-2 hrs. It is extensively plasma protein bound ( more than 90%). Peak level is achieved in 4-6 hrs. The plasma t 1/2 is 4 hrs and its duration of action is 16- 24 hrs.

Indications

Glipizide belongs to second generation sulfonylureas.It is indicated in: Non insulin dependant diabetes milletus[ NIDDM], maturity onset diabetes and in insulin resistant diabetes. Glipizide cause release of insulin from the pancreas in response to a meal, it does not increase the fasting insulin level. It also increases the tissue sensitivity to insulin and glucose uptake. It reduces gluconeogenesis.

Routes of Administration and Dosage

For Noninsulin dependant diabetes, oral dose: The drug is increased in stepwise fashion. It is started as 2.5 mg/day given 1/2 hr before breakfast. Then the dose is raised over a period of 2-4 weeks to 20 mg/day which is given in two divided doses, this

Contra Indications

Glipizide is contraindicated in the following conditions: pregnancy, lactation, pre-coma, coma, renal insufficiency, juvenile diabetes, impaired adrenocortical function, ketoacidosis .

Precautions

Precautions are to be taken in hypoglycaemia, stress, thyroid impairment, gastroenteritis and in conditions where there is caloric deprivation.

Interactions

The hypoglycaemic action of glipizide is enhanced by alcohol, sulphonamides, tri-methoprim, salicylates and NSAID's. Non-selective beta-blockers can mask the hypoglycaemic effect of glipizide.
